Group Life/AD&D Insurance

Benefit Summary

All information provided is in summary format.  For complete details of coverage, exclusions, limitations, etc., please refer to the summary plan document, certificate or master contract, not this online summary.

Class of Employees

All full-time hourly and salaried employees of Staker & Parson Companies, as defined by company policy, excluding temporary and seasonal employees.

Waiting Period

1st day of the month following the date he completes 3 months of continuous employment with Staker & Parson.

Basic Life Benefit

$50,000 - will reduce to 50% at age 70 and terminate at retirement

AD&D Rider

$50,000 - will reduce to 50% at age 70 and terminate at retirement.

Dependent Life

$2,000 for spouse and each child less than 19 years of age, students 19 years of age but less than 26 years of age.

Accelerated Death Benefit

Up to 50% of the Basic Life Insurance in force to a maximum of $25,000.  Employee must have at least $10,000 in Life Insurance in-force to qualify for this benefit.

Waiver of Premium

The Covered Person's premium payment will be waived on a monthly basis, beginning the first day of the month after the month he became Totally Disabled, if he:

  1. becomes Totally Disabled before age 60;

  2. remains Totally Disabled continuously for at least 9 consecutive months;

  3. gives proof of Total Disability, as required.
