Group Life/AD&D Insurance
Benefit Summary

Class of Employees |
All full-time hourly
and salaried employees of Staker & Parson Companies, as defined by
company policy, excluding temporary and seasonal employees. |
|
Waiting Period |
1st day of the month
following the date he completes 3 months of continuous employment
with Staker & Parson. |
|
Basic
Life Benefit |
$50,000 - will reduce
to 50% at age 70 and terminate at retirement |
|
AD&D Rider |
$50,000 - will reduce
to 50% at age 70 and terminate at retirement. |
|
Dependent Life |
$2,000 for spouse and
each child less than 19 years of age, students 19 years of age but
less than 26 years of age. |
|
Accelerated Death
Benefit |
Up to 50% of the Basic
Life Insurance in force to a maximum of $25,000. Employee
must have at least $10,000 in Life Insurance in-force to qualify
for this benefit. |
|
Waiver of Premium |
The Covered Person's
premium payment will be waived on a monthly basis, beginning the
first day of the month after the month he became Totally Disabled,
if he:
-
becomes Totally
Disabled before age 60;
-
remains Totally
Disabled continuously for at least 9 consecutive months;
-
gives proof of Total
Disability, as required.
