As a classified employee of the District, meeting the eligibility requirements, you will have options for medical and dental insurance, and vision, term life insurance and long-term disability benefits.  This Guide is designed to help you better understand your benefit options and the enrollment process.  It includes:

• A comparison of the medical plans

• An outline of the dental and vision plans

• Costs for Additional Life Insurance and Supplemental Life Insurance, and Critical Care

• Information on the Section 125 Flexible Spending Account Program

• Step-by-step instructions on how to complete your Benefits Enrollment Form

Also enclosed with this Guide are separate, but required enrollment forms:

• BCBS or Kaiser application for medical and vision

• BCBS or Kaiser application for dental coverage

• The Personal Choice Account® enrollment form

• OEA Choice Enrollment application for life insurance coverage
