New Employee Benefit
Orientation
Mann Mortgage LLC is pleased to be able to
offer a wide variety of benefits for eligible employees. This site
should provide you with all the information needed to help you enroll
in the benefits that will best serve you and your family.
Probationary
full-time employees, (working 30 or more hours per week),
are eligible for the Flex Benefits Plan, Medical, Dental, Vision,
Accident, Group Life, Voluntary Life, Short and Long Term
Disability, and Long Term Care insurance.
Probationary
part-time employees, (working 20 or more hours per week),
are eligible for Accident insurance.
Benefits are effective
on the first day of the month following completion of sixty (60)
calendar days of continuous employment with Mann Mortgage.
Employees who waive benefits may be required to wait until the next
plan specified enrollment date.
Domestic Partner
Eligibility: Effective January 1, 2007 domestic partners may be
covered under Mann Mortgage benefit plan. To be given
consideration, individuals must sign and satisfy all requirements of
an
Affidavit of Domestic Partnership. Open enrollment
rules and special enrollment rules are applicable to domestic
partners. Domestic Partners and their dependent children may not
qualify as dependents under Section 152 of the Internal Revenue Code
for tax purposes and therefore may not be able to participate in the
Flex Plan. Contact the HR Department if you have questions.
ENROLLMENT GUIDE:

Complete every benefit form
either enrolling or waiving coverage.

Complete the Beneficiary Card for Group
Life Insurance.

Fax enrollment forms to the Human
Resource Department at 406-751-6283.

Full and part time employees are eligible
to enroll in the 401(k) Savings Plan, 401(k) Roth Savings Plan and 529 College Savings Plan
following the completion of 3-months of continuous employment with
Mann Mortgage. Participants in the 401(k) plan must also be 18 years
of age. Quarterly enrollment dates are January 1, April 1, July 1 and
October 1. Employees may enroll any quarter, provided they have
fulfilled eligibility requirements. When you first become eligible,
you should receive enrollment kits for these benefits in the mail
approximately one month prior to the quarterly enrollment. You may
view plan information on the links provided on this website. However,
do not enroll online. Complete the enrollment form provided in
your enrollment kit and fax and then mail to the Human Resource
Department as directed above.
Any
inconsistencies between this information and the plan provisions will
be governed by the plan documents.
