Long Term Disability Plan - Regence Life and Health
Employees Only

November 1, 2008 - October 31, 2009

Eligibility
(Employees Only)

Class One:  Management and Certified Employees
Class Two:  Classified Employees

Elimination Period

90 days or to the end of accumulated sick leave, whichever is greater

Definition of Disability

Class One:  Long-Term Own Occupation (LTOC) with Residual
Class Two:  5 Year Own Occupation with Residual

Benefit Duration

To age 65 if disabled prior to age 60.  Period is limited if disability occurs after age 60.

Monthly Benefit

66.67% of basic monthly earnings, less any other income benefits

Monthly Maximum

$9,200

Monthly Minimum

Not less than $100 or 10% of the gross monthly benefit, whichever is greater

Pre-Existing Conditions Any disability that is related to a pre-existing condition which begins within 12 months after you become insured under the policy, will not be covered.

A pre-existing condition means any injury or sickness that you were treated, consulted, or received care for in the last three months prior to the day you become insured under the policy.  If you have received care in the three month period, you mush continue 12 months treatment free in order to be treated for that condition.  If you are currently insured with LTD coverage, you will not have to satisfy this requirement.
